Association of Cytomegalovirus Infection and Disease With Death and Graft Loss After Liver Transplant in High-Risk Recipients

In the transplant literature, data associating CMV with rejection and mortality are conflicting.  In this retrospective cohort of 227 CMV D+/R- liver transplant recipients, the authors used a combined endpoint of graft loss or death.  CMV infection and disease was reported in 40% and 19% of patients respectively within one year of transplant; this is well within previous reported incidence of CMV in the sero-mismatched population.  CMV infection was associated with a 2.24 fold increase risk of the combined endpoint (graft loss or death); CMV disease was also associated with a 2.73 fold increase in death or graft loss. This study provides further incentive for more aggressive preventative strategies towards CMV infection in this population.
