Mycophenolate mofetil has supplanted azathioprine in most immunosuppressive protocols since the initial randomized controlled trials in the early 1990′s showed a reduction in the incidence of acute rejection. More recent RCTs have cast doubt on whether this difference persists when used with more modern immunosuppression. This analysis of US registry data showed no difference in outcomes among tacrolimus-treated patients who received azathioprine or MMF. Given that the cost of MMF is ten-fold that of azathioprine, these results should be verified in a RCT.
