Better off living–the ethics of the new UNOS proposal for allocating kidneys for transplantation

This article, part of CJASN’s new section on public policy, critically analyzes the new policy proposal by UNOS to allocate deceased donors kidneys in the UNOS.  The authors examine several potential objections: 1. Is waiting time a fair way to rank order patients since there are disparities in gaining access to listing? 2. It is unfair to older adults to use age in allocating health care? 3. Does the limited accuracy of the formula used to estimate patient survival mean that the allocation scheme is unfair? 4. Will directing deceased donors preferentially to young adults result in lower live donor rates to these groups, and is this unfair? They also identify several “neglected ideas” not included in the proposal, including eliminating geographic priority, limiting access of non-US residents, abolishing multiple listing etc.
