Solid phase assays on the Luminex platform have become the standard for the detection and characterization of HLA antibodies. However, transplant professionals have struggled with interpretation of the highly sensitive Luminex assay with respect to cytotoxicity assays and clinical relevance. To tackle this problem The Transplantation Society asked a group of experts with extensive experience with laboratory and clinical understanding of HLA antibodies to prepare a set of concensus guidelines. The list of recommendations addresses three key areas: 1) Technical issues relating to the use of Luminex assays, 2) Interpretation of the assays in the pre-transplant setting, and 3) Application of the assays in the post-transplant setting. These recommendations will help to provide guidance in the use and clinical application of sensitive Luminex HLA antibody detection assays.
