This article and its accompanying editorial examine the UDCDD protocol in New York City, in which people who die from cardiac arrest out of hospital can be used as organ donors.  The article explains how the protocol was developed, and the involvement of government officials, content experts (including ethicists) and the public in the process. The accompanying editorial congratulates the authors on their thoughtful review and approach to the problem, but remains concerned that the UDCCD protocol may be seen as inconsistent with the current definition of circulatory death and may be seen by some as resulting in premature determination of death in patients who do not yet have permanent cessation of circulatory function.
