This study explores the hypothesis that patients with metabolic syndrome would develop more cardiac allograft vasculopathy (CAV) as a consequence of the endothelial dysfunction associated with this syndrome. The authors analyzed the relationship between the presence of metabolic syndrome at the time of heart transplantation and in the first 3 months with the development of CAV at 1 year after heart transplant confirmed by IVUS.  Individual components of the metabolic syndrome had less of an impact on the development of allograft vasculopathy.  The findings of this study suggest a personalized approach to screening for CAV may be adopted in this higher risk subgroup.
