Monitoring Infection with Epstein–Barr Virus among Seromismatch Adult Renal Transplant Recipients

PTLD is often serious and difficult to treat post-transplant.  Antiviral prophylaxis is often used in EBV D+/R- transplant patients and monitoring is done in many centers for early detection of viremia.  In this single-center cohort study, EBV mismatched kidney transplant patients were monitored.  A small number of patients with persistent viremia in the first year were given pre-emptive Rituximab therapy along with reduction in immunosuppression and this appeared to successfully prevent PTLD.  In addition, the few patients that were not monitored had much greater rates of PTLD.
