In recent years, more lung transplantation centres have performed lung transplants in high risk patients, including patients with high lung allocation (LAS) scores, requiring mechanical ventilation, or extracorporeal membrane oxygenation (ECMO) support pre-transplant. This review of the UNOS database identified that patients bridged to lung transplant with mechanical ventilation or ECMO had significantly worse short-term and long-term survival when compared to other high-risk recipients. The database shows outcomes of high risk patients have been improving over time, particularly in high volume transplant centres.
