Pancreas transplantation has traditionally been reserved for patients with type I diabetes, due to the belief that the insulin resistance seen in type II diabetes would not allow patients to come off exogenous insulin. In this analysis of US registry day, 9% of kidney-pancreas transplants went to patients with type II diabetes, and rates of death, kidney and pancreas graft failure were similar in type I and II patients after adjustment for other risk factors. A provocative editorial argues that pancreas transplantation  should be expanded to more patients with type II diabetes.
