Most registries show worse outcomes with pancreas-after-kidney (PAK) compared to simultaneous pancreas-kidney (SPK) transplants. In this single-centre, retrospective study, living donor renal transplants followed by pancreas transplants showed comparable results to SPK, despite the long time between kidney and pancreas transplants. Greater use of PAK may shorten time of dialysis for patient with a living kidney donor and increase overall transplant rates.
