Pediatric Heart failure and worsening renal function: Association with outcomes after heart transplant

Renal function has been shown to be a risk factor for death while waiting for heart transplant and following transplantation in the pediatric population. Little is known about the effects of a deterioration of function from the time of listing to transplantation. This study was designed to determine the effects of worsening renal function during listing on the outcomes post transplant. Data from the Organ Procurement and Transplant Network (OPTN) database was used for all children <18 years who underwent a heart transplant, between 1989-2009. 15% of children awaiting heart transplant developed worsening renal function with the majority considered to be mild. The results also showed that any degree of worsening renal dysfunction while awaiting transplant was associated with in-hospital mortality post transplant. What remains unclear is if strategies to prevent renal dysfunction would improve early post transplant outcomes
