The Istanbul Declaration was an international statement condemning the practices of organ trafficking and transplant tourism. This paper outlines an approach for physicians to counsel patients about the risks of commercial renal transplantation beforehand, physicians’ duties towards a patient who wishes to proceed with a commercial transplant, and their responsibilities once a patient returns seeking care. Although developed in the context of the Canadian health-care system, many of the principles advocated here are applicable in other countries.
