Should heart transplant recipients with early graft failure be considered for retransplantation?

This retrospective review of the UNOS database revealed the median survival in patients retransplanted within 90 days was only 1.6 years, compared with 10.5 years in primary heart transplant recipients. The trend in decline in organ donation in combination with decreased survival following retransplantation, raises ethical considerations regarding the decision to proceed with repeat transplantation after early graft failure. The efficacy and cost implications of temporary mechanical support in these cases are also discussed.
