This article is a must-read for anyone wanting to understand better why some countries enjoy higher donation rates than others and the importance of a multi-pronged strategy to increasing deceased donor rates long term.  This article clearly explains the “Spanish system” of organ procurement that is held up as an international example of success due to its extremely high rates of deceased donation.  Although people often credit the presumed consent legislation in Spain with this success, the authors explain that this law has not actually had much impact on donation rates. There are other aspects of the  system that actually deserve the credit, specifically the national organizational system, the introduction of physician transplant coordinators at both small and large sites and the system’s engagement with the Spanish public and media to keep donation in the public eye.
