Airflow obstruction on spirometry after lung transplantation is a common finding, with several potential causes. This study of 122 double lung transplant recipients examined the outcomes of this pattern on early post-transplant outcomes. Fourteen percent of recipients demonstrated an obstructive spirometry pattern, and these recipients had significantly lower FEV1, increased total lung capacity, and lower donor organ pO2 on 100% FiO2 before organ retrieval. Early airflow obstruction was also associated with an increase in incidence and earlier development of bronchiolitis obliterans syndrome.
