A recurrent theme at the annual International Society of Heart and Lung Transplant and the Heart Failure Society of America meetings is the need to consider further revision in the UNOS wait list criteria.  This paper reviews the justification and evolution of the current criteria through the era of mechanical circulatory support (MCS) and addresses weakness in the current system including potential discrimination against Stage D heart failure patients who are not best served with MCS.
