Subclinical Rejection in Stable Positive Crossmatch Kidney Transplant Patients: Incidence and Correlations

Increasing numbers of patients are receiving transplants after being desensitized to overcome a positive crossmatch or ABO-incompatibility with their donor. These patients are known to be at an increased risk of acute humoral rejection early post-transplant. This retrospective study shows that a significant number of these patients will have subclinical cellular or humoral rejection on protocol biopsies up to one year post-transplant. The use of protocol biopsies in this group of patients may identify those who would benefit from further treatment despite having stable renal function.
