Superior Preservation of DCD Livers With Continuous Normothermic Perfusion

Donation after cardiac death (DCD) liver transplantation has grown because of the discrepancy between the number of patients on the transplant waiting list and the number of available organs. Unfortunately, numerous studies have demonstrated increased biliary complications and lower graft survival with DCD liver transplants. Multiple groups are looking at the use of machine perfusion in order to improve results with DCD donors. The present study showed that normothermic machine perfusion with normothermic extracorporeal membrane oxygenation (NECMO) is superior to cold storage or the combination of NECMO and cold storage in a large animal DCD model. These results are very encouraging and will hopefully be tested in the clinical setting in the near future.
