Liver transplants rarely experience antibody-mediated rejection, and it has been suggested that a liver transplant can protect the kidney if a simultaneous transplant is performed. In this single-center study, patients who received a simultaneous liver-kidney transplant had a higher risk of acute rejection, but similar renal function and patient and graft survival, whether or not […]

While sirolimus has some features that make it an attractive immunosuppressant, it is less effective at preventing acute rejection than calcineurin inhibitors (CnI). An alternate strategy is to use CnI early post-transplant and then switch to sirolimus at some point post-transplant. In this randomized controlled trial, patients with good graft function and no other contraindications […]

Immunosuppression Regimen and the Risk of Acute Rejection in HIV-Infected Kidney Transplant Recipients

Renal transplantation for carefully selected HIV-positive patients has become well-accepted over the last decade. There remain concerns about the risk of overimmunosuppression in this group, due to their pre-existing immunodeficiency due to HIV infection. In this US registry study, HIV-positive patients actually had a higher risk of acute rejection than HIV-negative recipients, but this increased […]

The gold standard fro diagnosing acute rejection remains biopsy. A non-invasive test that could reliably diagnose rejection would be an important step forward. In this study, a set of three different RNA molecules in the urine of renal transplant recipients showed a high degree of sensitivity for acute cellular rejection, and distinguished it from antibody-mediated […]

Post-transplant, mycophenolic acid (MPA) dose is frequently reduced because of side effects or toxicity. However, overall MPA levels are higher in patients on tacrolimus as opposed to cyclosporine. In this prospective study of patients who were also on tacrolimus, MPA dose reduction or discontinuation was associated with a higher risk of subsequent acute rejection. The […]

Perhaps one of the most intriguing concepts in modern medicine is the potential for personalized approaches to medical therapy. Currently, there are no biomarkers in clinical use that can predict, before transplantation, the occurrence of acute rejection. This Canadian group of researchers originating from the PROOF centre in British Columbia has identified a biomarker panel […]

VItamin D deficiency has been associated with a variety of illnesses. More recent evidence shows a possible effect on lung function. Lung transplant recipients have a high incidence of vitamin D deficiency. This retrospective study identified an association between vitamin D deficiency near the time of transplant, and increased rates of acute rejection and infection.

High rates of rejection in pulmonary allografts compared to other solid organ transplants may be related to constant exposure to the outside world. This study examined the impact of daily variations in air pollution on the outcome of lung transplant by correlating histologic evidence of acute rejection with daily particulate matter values at the home […]

Antibody mediated rejection (AMR) is a well described phenomenon in kidney and heart transplant but its presence and impact on intestinal transplantation is unclear. AMR might contribute to the severity and relatively high rate of steroid resistant acute rejection in intestinal transplantation. This prospective study measured and correlated donor specific antibodies (DSA) levels to episodes […]
