This randomized controlled trial assessed whether sirolimus would prevent further skin cancers in patients who have had a first squamous-cell skin cancer post-renal transplant. Patients were either continued on their calcineurin inhibitor or were switched to sirolimus. Freedom from a second skin cancer was longer with sirolimus. Switching to sirolimus earlier after a fist skin […]

Randomized Controlled Trial of Sirolimus for Renal Transplant Recipients at High Risk for Nonmelanoma Skin Cancer

Several randomized controlled trials have shown lower rates of non-melanoma skin cancer among patients who receive sirolimus from the time of transplant. This study randomized previously transplanted patients who had previously had at least one skin cancer to either continue on their usual immunosuppression or change their calcineurin inhibitor to sirolimus. There were fewer new […]

Meta-analyses in the general population have suggested that ACE-inhibitors and angiotensin receptor blockers may increase the risk of cancer, particularly lung cancer. In this report from the international Collaborative Transplant Study, use of ACE-I or ARB as associated with a statistically significant increased risk of lung cancer, but only in patients with a smoking history. […]

Spectrum of Cancer Risk Among US Solid Organ Transplant Recipients

Malignancy is a well-recognized complication of organ transplantation. Most studies have focused on renal transplant recipients. This study uses US registry data to examine the risk of cancer among recipients of kidney, liver, lung, and heart transplants. While the risk of cancer was increased in all groups of patients, there were variations in cancer risk […]

Monoclonal gammopathy of unknown significance (MGUS) is often benign but can progress to myeloma in some cases. Whether immunosuppression increases the risk of progression is unknown. In this retrospective study, MGUS was not associated with progression to myeloma, amyloid or lymphoma post-transplant. MGUS should not be considered a contraindication to transplant.

This multicenter study (based on 32 US centers contributing to the Cardiac Transplant Research Database), provides detailed skin cancer risk information about cardiac transplant recipients that is less influenced by geography or ethnic factors than single-center data.  Such risk factor data can help direct preventive and treatment strategies to the most at-risk individuals.

This article examines the ethical considerations around using living donors for liver transplantation for recipients with hepatocellular carcinoma. The authors summarize the discussion and recommendations of a 2010 consensus conference on this issue and answer three questions:  Why is live donation necessary at all and is it different for patients with HCC? What is double […]
