This prospective study demonstrated that the incidence of ventricular assist device related infection continues to burden approximately 20% of implanted patients in the era of smaller continuous flow devices. There was an associated increase in one year mortality.

Post-transplant, there is no ideal measure of predicting how immunosuppressed an individual recipient is. Fernandez-Ruiz et al. assessed complement (C3 and C4) levels at 0,1,6 months post-kidney transplant in 270 patients. Those with C3 hypocomplementemia at month 1 had greater incidences of overall infection, bacterial infection and fungal infection up to month 6 post-transplant. This […]

This multicenter, randomized controlled trial assessed the efficacy, safety and incidence of CAV in de novo heart transplant patients receiving everolimus at two doses (1.5 mg/day or 3.0 mg/day in divided doses) with reduced-dose cyclosporine versus MMF with full-dose cyclosporine. The high dose everolimus group was terminated due to increased mortality. The lower dose everolimus […]

The transition from a functioning renal transplant to dialysis has been recognized as a high-risk period. In this study using data from the multinational DOPPS study, patients with a failed renal transplant had worse outcomes than wait-listed dialysis patients. A major cause of death among patients with a failed graft was infection.

Up to one-third of transplant recipients can develop hypogammaglobulinemia. However, it is controversial whether the presence of hypogammaglobulinemia leads to an increased risk of infections. Further, many transplant centers do not do routine monitoring for this. The authors of this paper prospectively monitored 226 kidney transplant patients at 0, 1, 6 months post-transplant. At one […]

VItamin D deficiency has been associated with a variety of illnesses. More recent evidence shows a possible effect on lung function. Lung transplant recipients have a high incidence of vitamin D deficiency. This retrospective study identified an association between vitamin D deficiency near the time of transplant, and increased rates of acute rejection and infection.

Pneumocystis jiroveci pneumonia is a well-recognized complication after transplantation, and current guidelines recommend prophylaxis for three-six months post-transplant. In this study, nine patients who develop Pneumocystis pneumonia a median of 19 months post-transplant were compared to a group of control patients. Cases had significantly lower lymphocyte counts than controls, as well as lower CD4 counts, […]
