The impact of MELD/PELD revisions on the mortality of liver-intestine transplantation candidates

Patients listed for liver-intestine transplant experience higher mortality while waiting for an organ compared to patients listed for liver transplant only. As a result UNOS implemented a new policy in 2005 and revised it again in 2007, in which adult and pediatric patients waiting for intestine-liver transplant are granted extra MELD and PELD points to improve the survival of listed patients. This paper review the impact of the new policy on the mortality rate of patients listed for liver-intestine transplant. The MELD / PELD modifications led to improved survival on the waiting list but mortality disparity remains for liver-intestine candidates compared to liver only.
