In order to ascertain methodological gaps in assessing adherence in the literature this prospective study of adult organ transplant recipients compared 79 heart, 55 liver, and 104 lung recipients . Non-adherence was measured using self-report, collateral report, blood assays, electronic monitoring, and their combinations. In the overall sample, depending on the method used, Non-adherence ranged from 23.9% to 70.0%. At inclusion, significant non-adherence differences between organ groups were reported via self- and collateral report. This study highlights the discrepant results that may be obtained by varying methods utilized to evaluate non-adherence, with an extremely wide range of non-adherence rates. A standardized way to assess and report adherence is strongly warranted and required to address this important transplant issue.
